breath You WILL be turned away, due to the current COVID-19 Pandemic.

Please call and let us know before your scheduled appointment time and we will convert

your appointment to a televisit.

INSTRUCTIONS FOR ALL-VISITS:

  • Step by step instructions for visits 
    • New Patient visits (virtual – both Sharon Krieger and Jeanne Botz APRNs): 
    • Confirmation Email is sent when scheduling appointment, with link (via email) 
    • Fill out smart form new patient paperwork sent from Kareo via email – check spam/junk folder if not in your inbox. 
    • If you do not have a computer or printer, we can make arrangements to mail the paperwork to you beforehand, so it can be filled out at home and returned to the office before the appointment date. 
    • Expect a phone call from reception for payment (if applicable/self-pay) and another from patient assistant for pre-appointment check in (vitals, medication etc. questions)- the patient assistant may also be on your virtual appointment to obtain this information.  
    • Link is sent from Kareo about one hour before appointment time.  
    • Please check your texts, email (including spam/junk folder) 
    • Call the office at extension 1 if not received 15 minutes before appointment so it can be resent.

    • How to access your telehealth (video visit): 
    • Link is sent at time of scheduling via email (please check spam/junk folder) and again approximately 1 hour before appointment time via text message. 
    • If you need the link resent, please call the office 10-15 minutes before your appointment. 
    • Click link – 5 minutes before your appointment – via your device (laptop, cell phone, tablet) and enter your birthdate.  
    • Make sure your camera, mic and speakers are turned on/up.
